At Southside Center, the Vietnamese Health Clinic helps elders and their families manage diabetes, heart disease and other chronic conditions. The clinic serves as a primary care provider for many elders in the community. Children also receive school physicals, while an annual health fair serves hundreds. The clinic is operated in partnership with clinical professionals from Institute for Family Medicine, while the bilingual staff at SFCS Southside Center provides healthcare-related case coordination, including transportation, help making external appointments, medication management, and education.
